Veterinarian Consultation for Hospice Services

- Education and options for on-going comfort care

- Medication management, prescription refills, options for comfort care and entry into our hospice program.

CONTINUATION OF INITIAL CONSULTATION

- Current Hospice Clients Only
*If during the above Veterinarian hospice consultation (less than 60 minutes) you determine that it is time to euthanize your pet, we will change the visit to the Euthanasia package you choose, invoice you for the difference and perform the euthanasia service/package of your choice. If the Doctor has been with you over 60 minutes and then you decide to proceed with euthanasia, you will be charged for the hospice and euthanasia appointment.
Acupuncture Services
Initial Consultation, Examination, and 1st treatment $300.00
- Initial consultation, complete examination and 1st acupuncture treatment

Follow Up Acupuncture – (2nd Visit Acupuncture Previous Clients Only) $225.00

Included in our fee is a 25-mile driving radius from Tatum Boulevard and the 101 Freeway. Please google distance you can use 21001 N Tatum Blvd. Phoenix, Arizona 85050. We are able to travel outside of the area for below additional fees:

After Hours Fees:
Monday Through Thursday 4pm-8pm $50.00
Friday 3pm-8pm $75.00
Saturday and Sunday 8am-8:15pm $75.00

Overweight Fees Apply as follows:
Additional Fee of $25.00 for pets 100LB to 120LB
Additional Fee of $50.00 for pets over 121LB to 150LB
Additional Fee of $75.00 for pets over 151LB to 175LB
Additional Fee of $300.00 for pets over 176LB
